Test Accommodations Request
Students with documented disabilities who are eligible for test accommodations through Disability Support Services must complete this form to request such services. Please schedule your exam with the DSS office and submit the form below or give the completed form to your faculty.
This accommodation requires:
- the student to submit the request form to the instructor 10 days before an exam,
- both the student and the instructor to complete their sections of the form,
- the form to be received by DSS at least 5 days prior to each desired test, and
- the test to be submitted to DSS at least 24 hours prior to the scheduled testing session.
